We deliver an exemplary model focused on person-centred holistic and flexible access including 24/7 direct admissions.
We are committed to offering support to people, their families and carers who are seeking person-centred palliative and end-of-life care within the Geelong and wider region. Our services include overnight care, a bespoke day program, consultations, community outreach and equipment loan bank. Care occurs in a unique, home-like environment with best practice clinical care including support from trained palliative care volunteers.
The focus of person-centred palliative care is on the quality of life, symptom management and comfort.

Overnight Care
Individuals with a life-limiting diagnosis are eligible for admission to Anam Cara House Geelong for assessment, symptom management and end-of-life care. We welcome anyone regardless of their geographic location.
Our care is person-centred and encompasses the physical, emotional and spiritual needs of our guests. Care is provided to our guests by nurses and specialists palliative care physicians who are on site 7 days a week (including public holidays) and available on call when they are off site.
We have a spiritual care and wellbeing coordinator who works with guests and families, and access to multiple allied health disciplines.
Our palliative care trained volunteers offer support and companionship.
Our care extends beyond the dying event, with bereavement support and an annual memorial service to honour their memory.

Living Well Day Palliative Care Program
Open now — 4 days per week
Our Living Well Day Palliative Care Program offers clinical assessment, symptom monitoring and management, spiritual care, education, as well as the opportunity to enjoy a range of activities such as art, music and pet therapy, gallery and beach visits, coffee mornings and much more.
Registered nurses and specially trained volunteers are present to provide support and care.
Our program not only offers individuals the opportunity to socialise with other guests and the people of Anam Cara House Geelong, but it also allows family members and caregivers to have some valuable time to themselves.

Consultations
Our specialist palliative care physicians and spiritual care and wellbeing coordinator see people in our consulting suites at Anam Cara House Geelong for assessments, symptom management, care coordination with other healthcare providers, and assistance with advanced care planning.
The goal is to enhance comfort and support guests and their families throughout their journey.

Community Outreach
Our Community Outreach Nurse offers support and advice to guests/family with a life-limiting illness in their own home.
They can assist with assessment, referral and care navigation.
We also have an Equipment Loan Bank including electric beds, shower stools, chairs for showers, wheelchairs, electric lift chairs etc. Equipment can be requested by Allied Health practitioners, where Anam Cara House Geelong will deliver directly to the person. If a request is made directly by the person or carer, our Community Outreach Nurse will visit to assess and discuss suitability for the equipment and provide education.
Items that are no longer required are paid forward to those in need in the community through our ‘Pay It Forward Bank’ initiative.

Jeanette & Chris - The Carer's Story
Anam Cara provides support and care for all of our guests, but let's not forget how important it is to help the hard-working carers, too.
24 hours 7 days a week with her partner Chris, Jeanette was never getting a break as his full-time carer. Anam Cara ticked all the boxes. When Chris goes to Anam Cara he is almost euphoric. Jeanette turns around and walks away with her big list of things to do, she never turns around and looks back – she knows he is safe.
